* Some times insurance companies require the client to submit the reciept themselves before being reimbursed; payment is required at the end of treatment by the client in this case.
Clients are responsible to pay any outstading balances not paid by insurer. Missed appointment or cancellation fees are not covered by insurance comapnies and are to be paid by the client.
It is recommended that prior to booking any appointments for first time to call your insurance company and ask what their policies are. (i.e: do they require a Doctors referral for coverage?, is there a deductible?, do they cover 100%, 80%, or 50% of treatment?).
